A lot of data corruption has been happening recently, and it's been discovered that those very same 256 GB Samsung SD-cards are the issue (or rather, fake ones that are almost indistinguishable from the real ones). My only advice is to use different sd cards, like San Disk, and you should probably buy them from a brick-and-mortar store.
